Is your patient in a crisis situation or acutely unwell? NO

Is a review of Item 291 appropriate to support the care of your patient? YES

Will you be responsible for ongoing care for the patient? YES

Is the assessment for a third party (eg: Court, WorkCover, Superannuation)? NO

Are you referring a new patient for an initial psychiatric assessment? Yes

Has the patient had a consultation with this psychiatrist in the proceeding 24 months? No

Does continued psychiatric care provide suitable support for your clinical care of the patient? YES

Is the assessment for a third party (eg: Court, WorkCover, Superannuation)? NO

Are there any risk factors that make telehealth care inappropriate for this patient? NO

Do you request the appointment to be bulk billed through medicare? NO

If the criteria are not met, consider MBS item 291 as described above.
